Terms and Conditions

Welcome to Performance X Medicine's mobile concierge physical therapy practice. By engaging our services, you agree to abide by the following terms and conditions:

1. Monthly Packages:

• The cost of monthly packages is non-refundable if the patient does not utilize all their visits.

• Refunds are only applicable if the physical therapist cancels the appointments.

• Monthly packages entitle the patient to a pre-determined time slot, guaranteeing two visits per week. If the patient cannot make these scheduled times, they are not guaranteed a visit on that day.

2. Travel Fees:

• Servicing Beverly Hills/West Hollywood and adjacent municipalities.

• Travel times exceeding 30 minutes incur a driving fee of $37.5 per 15 minutes.

3. Concierge X Medicine Services:

• Initial Evaluation: $250

  • Conducted at the location of your choice, focusing on understanding the source of pain or movement dysfunction, identifying underlying causes, outlining a roadmap for rehabilitation, and setting expectations for the rehab process.

• Follow-Up Visits: $200

  • Custom-tailored sessions, typically 1-2 times per week, including manual therapy, therapeutic exercise, performance-specific exercises, and specialty interventions.

• Monthly Package: $1800

  • Includes three sessions per week with a 25% discount on follow-up sessions.

  • Individual sessions are refundable only if the therapist cancels the appointment.

4. Payment:

• All major credit and debit cards are accepted through PayPal invoicing.

5. Appointment Application:

• Clients are required to provide their name, email, date of birth, zip code, and any applicable referral code when scheduling appointments.

6. Customer Reviews:

• Testimonials provided by satisfied customers on Google Reviews attest to the quality and effectiveness of our services.

7. Confidentiality:

• We prioritize the confidentiality of all patient information and adhere to HIPAA regulations. Any personal or medical information shared during sessions will be kept strictly confidential.

8. Scope of Services:

• Our services are limited to physical therapy treatments and interventions as determined by the licensed physical therapist. We do not provide medical diagnoses or treatment beyond the scope of physical therapy practice.

9. Code of Conduct:

• We expect all clients to behave respectfully and courteously towards our therapists and staff. Any behavior deemed inappropriate, abusive, or threatening may result in termination of services.

10. Cancellation Policy:

• Clients must provide a minimum notice period for cancellations or rescheduling of appointments, typically 24-48 hours in advance. Failure to provide adequate notice may result in a cancellation fee.

11. Liability Waiver:

• Clients acknowledge and accept the inherent risks associated with physical therapy treatments. Performance X Medicine and its therapists will not be held liable for any injuries, accidents, or adverse reactions that may occur during or as a result of treatment sessions.

12. Minors:

• For clients under the age of 18, a legal guardian or parent must be present during all sessions and provide consent for treatment.

13. Insurance:

• Clients are responsible for verifying coverage with their insurance provider. Performance X Medicine does not guarantee reimbursement from insurance companies and is not responsible for any denied claims.

14. Termination of Services:

• Performance X Medicine reserves the right to terminate services for any reason, including but not limited to non-compliance with treatment plans, failure to adhere to payment terms, or disruptive behavior.

15. Modifications to Terms:

• Performance X Medicine reserves the right to modify or update these terms and conditions at any time. Clients will be notified of any changes via email or on our website.

16. Governing Law:

• These terms and conditions shall be governed by and construed in accordance with the laws of [Jurisdiction], without regard to its conflict of law provisions.

17. Severability:

• If any provision of these terms and conditions is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
